To register, please return this completed form to Blakemore Children's Center with the appropriate application fee. The application fee is non-refundable. When your application is received, you will be contacted when space is available and informed about the registration process.

Preschool: $75 one-time fee

Infant/Toddler: $25 w/ application form $50 at enrollment

While unable to guarantee start dates, we will do everything possible to meet your needs. Enrollment is based upon availability and is subject to BCC enrollment guidelines
